| You can register for the Basic Floral Design Distance Learning
Program, On-Line or by Mail. |

| To register On-Line first print all pages of the enrollment form
listed above. Fill out the enrollment form completely. Sign and mail your
completed enrollment form. Then click on your preferred payment plan listed below
and pay On-Line using either Master Card or Visa. We will match your payment to your
enrollment form and send you an enrollment confirmation by both e-mail and regular
mail. You will also receive an e-mail asking you for sizes, containers and color
choices. Your Orientation, Tool Kit and Lesson 1 will be shipped within 7 days of
receipt. |

| To register by mail first print all pages the enrollment form for
that campus. Fill out the enrollment form completely. Sign and mail your enrollment
form with payment in the form of check or money order. We will send you an
enrollment confirmation regular mail. Your Orientation, Tool Kit and Lesson 1 will be
shipped within 7 days of receipt. |

| Full Payment, $1,345.00. If you choose to pay for the program
in full at the time of enrollment, you will receive the Tool Kit for FREE, a $155
value. |

|
| Partial Payment. A payment of $549 is due with the enrollment.
Three additional payments of $317 each are then due. The first $317 payment
is due 30 days after enrollment acceptance. The next $317 payment is due 60 days after
enrollment acceptance. The final $317 payment is due 90 days after enrollment
acceptance. |
